UNDERSTANDING HORMONAL CHANGES

What's actually happening to your body?

Starting in your late 30s or early 40s, your ovaries gradually produce less estrogen, progesterone, and testosterone. This transition, called perimenopause, can last 7-10 years before menopause (defined as 12 consecutive months without a period). During this time, hormone levels don't just decline. They fluctuate unpredictably, which is why symptoms can feel so erratic.
Estrogen influences over 400 functions in the female body. It regulates body temperature, supports bone density, protects cardiovascular health, maintains skin elasticity, fuels brain function, and governs mood through its relationship with serotonin and dopamine. When estrogen becomes unstable or deficient, the effects ripple across virtually every system.
Progesterone, often called the "calming hormone," supports sleep, reduces anxiety, and protects the uterine lining. Declining progesterone is frequently the first hormonal shift women experience, often years before estrogen drops.
These changes are a normal part of aging, but the symptoms they cause are not something you have to endure without help. Bioidentical hormone replacement therapy is the most effective, evidence-based treatment available for managing perimenopause and menopause symptoms.

WHAT HRT CAN DO FOR YOU

The benefits of balanced hormones

Check any symptoms you're currently experiencing. If you identify with 3 or more, a hormone evaluation may be your next step.

🌡️
Relief from hot flashes & night sweats
HRT is the most effective treatment for vasomotor symptoms. Most women experience significant reduction in hot flash frequency and severity within the first 2-4 weeks of treatment.
🧠
Restored mood & emotional balance
Estrogen plays a critical role in serotonin production and mood regulation. Balanced hormones help reduce anxiety, irritability, and the emotional volatility that can feel so disruptive.
🌙
Better sleep quality
Night sweats and hormonal fluctuations are among the most common causes of sleep disruption during perimenopause and menopause. HRT addresses the root cause so you can sleep through the night again.
💜
Improved libido & intimate comfort
Declining estrogen and testosterone both impact sexual desire and comfort. Our protocols address vaginal dryness, low libido, and arousal with targeted hormone and peptide therapies.
🦴
Protection against bone loss
Estrogen is essential for maintaining bone mineral density. HRT has been shown to reduce fracture risk significantly, especially when started within 10 years of menopause onset.
Sharper mental clarity & focus
Many women describe perimenopause brain fog as one of the most frustrating symptoms. Restoring hormone balance supports cognitive function, memory, and the ability to think clearly again.
TREATMENT OPTIONS

Your protocol, designed around your body

Every treatment plan at HexisMD is personalized based on your bloodwork, symptoms, life stage, and goals. Here are the tools our physicians use to restore your hormonal balance.

Bioidentical Estradiol
CORE TREATMENT
Plant-derived estrogen that is structurally identical to the estrogen your body naturally produces. Available as patches, creams, or oral formulations. The foundation of most female HRT protocols.
Bioidentical Progesterone
ESSENTIAL
Essential for women who have a uterus, progesterone balances estrogen and protects the uterine lining. Also supports sleep quality and has natural calming effects. Typically taken orally at bedtime.
Testosterone therapy (low-dose)
LIBIDO & ENERGY
Women produce testosterone too, and declining levels contribute to low libido, fatigue, and reduced muscle tone. Low-dose testosterone can restore drive, energy, and body composition.
DHEA
A precursor hormone that supports both estrogen and testosterone production. Can improve vaginal health, skin elasticity, and overall vitality. Available as oral supplements or vaginal inserts.
Thyroid optimization
OFTEN OVERLOOKED
Thyroid dysfunction is extremely common in perimenopausal and menopausal women and can mimic or worsen hormonal symptoms. We test and optimize thyroid function as part of every protocol.
Peptide therapy
ADVANCED
Targeted peptides to support collagen production, fat metabolism, sleep quality, and cellular repair. A powerful complement to hormone therapy for anti-aging and overall wellness.

FREQUENTLY ASKED QUESTIONS

Everything you need to know about HRT

Is hormone replacement therapy safe for women?

When prescribed appropriately and monitored by qualified providers, HRT is considered safe and effective for most women. The Women's Health Initiative study caused widespread fear about HRT in 2002, but subsequent research has clarified that the risks were overstated, particularly for women who start HRT within 10 years of menopause onset or before age 60. Bioidentical hormones, which we use at HexisMD, have a favorable safety profile. We evaluate every patient individually and monitor bloodwork regularly.

What is the difference between bioidentical and synthetic hormones?

Bioidentical hormones are derived from plant sources and are structurally identical to the hormones your body naturally produces. Synthetic hormones, like those used in older HRT formulations, have a slightly different molecular structure. Many providers and patients prefer bioidentical hormones because the body recognizes and metabolizes them more naturally. At HexisMD, we exclusively prescribe bioidentical hormone therapy.

When should I start HRT?

The ideal time to start is during perimenopause or within 10 years of your last menstrual period. Starting during this window provides the greatest benefits and the lowest risk profile. However, women who are further from menopause may still benefit from certain therapies. The best approach is to get your hormone levels tested and discuss your symptoms and health history with one of our providers.

Will HRT help me lose weight?

Hormonal imbalances, particularly declining estrogen and thyroid dysfunction, contribute significantly to midlife weight gain. While HRT is not a weight loss medication, restoring hormonal balance can improve metabolism, reduce cortisol-driven belly fat, and make it easier for your body to respond to diet and exercise. Many of our patients notice body composition improvements as part of their overall treatment plan.

Can I take HRT if I have a family history of breast cancer?

A family history of breast cancer does not automatically disqualify you from HRT, but it does require careful evaluation. Current evidence suggests that bioidentical estrogen combined with micronized progesterone carries a lower risk than older synthetic formulations. We assess every patient's individual risk factors, including family history, breast density, and genetic markers, to determine the safest approach for you.

How long can I stay on HRT?

There is no universal time limit for HRT. Many women benefit from ongoing treatment for years or even decades. The decision to continue, adjust, or discontinue HRT is made collaboratively between you and your provider based on your symptoms, health status, and evolving research. We reassess regularly to ensure your protocol remains optimal for your needs.

Will HRT help with my anxiety and depression?

Hormonal fluctuations during perimenopause and menopause are a well-documented cause of mood changes, including anxiety and depression. Estrogen directly influences serotonin and other neurotransmitters. Many women experience meaningful improvement in mood stability after starting HRT. However, HRT is not a replacement for mental health treatment. If you are experiencing severe anxiety or depression, we may recommend a combined approach.

Do you offer telemedicine consultations?

Yes. We serve patients across Florida and 45+ states via secure telemedicine. From your initial consultation to follow-up visits, everything can be done from home. We order bloodwork at a lab near you, review results via video consultation, and ship your prescribed treatment directly to your door. We also have an in-person clinic in Palm Springs, FL.
