Is testosterone replacement therapy safe?

When properly prescribed and monitored, TRT has a strong safety profile. The landmark TRAVERSE trial (2023), published in the New England Journal of Medicine, confirmed that testosterone therapy does not increase cardiovascular risk. In February 2025, the FDA removed the black box cardiovascular warning from all testosterone products based on this evidence. How do I know if I have low testosterone?

Common symptoms include persistent fatigue, decreased sex drive, difficulty gaining muscle, unexplained weight gain, brain fog, mood changes, and poor sleep. However, these symptoms can also be caused by other conditions. The only way to confirm low testosterone is through a blood test measuring total and free testosterone levels, ideally drawn in the morning when levels are highest. We run a comprehensive panel that includes testosterone, estradiol, thyroid markers, CBC, metabolic panel, and more.

What testosterone level is considered 'low'?

The generally accepted threshold is a total testosterone level below 300 ng/dL, though many men experience symptoms at levels between 300-450 ng/dL. The Endocrine Society and American Urological Association both emphasize a symptom-driven approach, meaning if your levels are borderline but your symptoms are significant, treatment may still be appropriate. We evaluate the full clinical picture, not just a single number.

Will TRT affect my fertility?

Exogenous testosterone can suppress sperm production. For men who want to preserve fertility, we offer protocols that include HCG (to maintain testicular function) or alternatives like enclomiphene (which boosts natural testosterone production without suppressing sperm). If you're planning to have children now or in the future, tell us during your consultation, we'll design your protocol accordingly.

How long until I feel results?

Most patients begin noticing improvements in energy, mood, and sleep within the first 2-4 weeks. Sexual function typically improves by weeks 3-6. Body composition changes; increased muscle, decreased fat, develop more gradually over 3-6 months of consistent treatment. Full optimization usually occurs around the 6-month mark.

Is TRT a lifetime commitment?

TRT is generally a long-term therapy. If you stop, your testosterone levels will return to their previous baseline. However, this is your choice, some men use TRT for a period and then transition to other approaches. What are the possible side effects?

Potential side effects include elevated red blood cell count (polycythemia), acne, fluid retention, and changes in mood. These are typically manageable with proper dosing and monitoring.
